Performance Appraisal Process/Form

DGS Policy requires all employees receive a written performance appraisals annually. The Individual Development Plan (STD 637) is the standard document for employee appraisals. Generally, the immediate supervisor is responsible for completing the IDP. The annual due date for an IDP is during the employee's birth month. IDPs are retained in the employee's Official Personnel File (OPF).

Purpose of an IDP/Performance Appraisal

  • Promotes communication between supervisors and employees
  • Creates a historical record of the employee's performance (to assist in promotional opportunities, transfers, MSA's etc.)
  • To review the employees performance, reinforcing what you want repeated and addressing areas that need improvement
  • Improve job performance now, plus promote job development for the future
  • Opportunity to recognize the employees excellent performance

Drafting the IDP

1. Download the performance appraisal form Individual Development Plan STD-637

2. Gather:
- Job duty statement
- Written job expectations (if available)
- Previous IDP and appraisal
- Work rules and procedures
- Employee's Bargaining Agreement MOU
- Employee Performance Log
- Any current disciplinary memos


3. Evaluate the Performance - Before filling out the appraisal form, take a moment to evaluate the data you gathered in step 2 above.

4. Provide Written Comments on the Appraisal Form:
- Provide specific examples of the employee's performance that support the rating category selected.
- Be sure to give details on level of supervision and how long they sustained the performance.
- Be Consistent, clear and factual
- Written comments should be consistent with IME category checkmarks.
